For some reason when I create form with a database, I get an error.

contact_2 is required and unique field

how can I fix this

Hello,

The Identifier field means that it can be used to detect the duplication of the data , it should hold the field name that contains the unique content, looks like you added invalid field name, are you sure its on your form deisgner
